Community Care's 'Health Home' Implementation Featured in Mental Health Weekly

The October 10 issue of Mental Health Weekly (MHW) highlighted Community Care's recent implementation of the health home model at two locations in Pennsylvania, in an effort to reduce high morbidity and mortality rates among consumers with serious mental illness. The model uses care coordination, peer support, and tools for both consumers and providers to help with wellness issues.

The MHW article covers program structure for the health homes, as well as discussing staff training, funding, and program evaluation.
