1999 - Southwest HealthChoices Implementation
Community Care was selected to participate in Pennsylvania's HealthChoices, a new integrated health care delivery system designed to provide Medical Assistance recipients with medical, psychiatric, and substance abuse services. Our first HealthChoices contract implementation was in the Southwest HealthChoices region and consists of Allegheny County.
2001 - Lehigh-Capital HealthChoices Implementation
Community Care implemented our Lehigh-Capital HealthChoices region contract. Our Lehigh-Capital HealthChoices counties include Adams, Berks, and York.
2004 - Southeast HealthChoices Implementation
Community Care implemented our Southeast HealthChoices region contract. This contract consists of Chester County.
2006 - Northeast HealthChoices Implementation
Community Care implemented our Northeast HealthChoices region contract. Our Northeast HealthChoices counties include Lackawanna, Luzerne, Susquehanna, and Wyoming.
2007 - North Central HealthChoices Implementation
Community Care implemented our North Central HealthChoices region contracts. The State option contract includes Bradford, Cameron, Centre, Clarion, Clearfield, Columbia, Elk, Forest, Huntingdon, Jefferson, Juniata, McKean, Mifflin, Montour, Northumberland, Potter, Schuylkill, Snyder, Sullivan, Tioga, Union, Warren and Wayne Counties. The County option contract consists of Carbon, Monroe, and Pike Counties.
2011 - Northwest HealthChoices Implementation
Community Care implemented our Northwest HealthChoices region contract. This contract consists of Erie County.
