Another Step on the Road to Recovery

Community Care has developed a program to enhance services for adult members 18-64 years of age. The program is called Steps of Hope. The program represents our commitment to the recovery model.

The program has been in effect since July 1st, 2008. It is based on evidence-based clinical practice guidelines, member input, and expert consultation. The goal of the program is to improve the lives of the members we serve. We hope to do this by encouraging members to embrace wellness and recovery planning.

The program is currently offered to members with a diagnosis of major depressive disorder or schizophrenia. Members receive interventions that are helpful to the member such as educational mailings every three months. Interventions, to mention a few, focus on talking to your doctor about yourself in general, about your medication, and your goals and what’s important to you. Members will also be encouraged and assisted to write a Wellness Action and Recovery Plan. Some members may receive supportive phone calls from staff. All members are welcome to call the Steps of Hope toll-free number at 1-866-225-8044 for questions, assistance, or if you think you would like to be in this program.

Staff is available Monday through Friday from 8 a.m. to 4 p.m., for non-emergency inbound calls. Calls are answered by licensed professional nurses from the Steps of Hope staff. After hours, onsite Customer Service Representatives will take calls. Licensed professional Care Management staff is available 24/7 for emergency calls and questions.
