Coordination of Care

Community Care measures how many of our providers are coordinating care with other people who play a role in our members’ treatment. This means that your behavioral health provider should be talking with your physical health provider or other behavioral health providers that are treating you. You have to give permission for your providers to talk to each other. Please consider letting your providers talk to each other.

When your care is coordinated:

  • You can be sure your medications are safe to be taken together.
  • You can avoid having the same tests repeated.
  • It can help you in your recovery.

If your child is in treatment, coordinating care means that all child-serving systems involved with your child’s care should be communicating. Child-serving systems can include schools, Children Youth and Families, Juvenile Probation Office and the Mental Health/Mental Retardation office.

Community Care works to improve the coordination of care by asking members to give their providers permission to talk. We also ask providers to talk to members about how important it is to coordinate their care.
