Getting Timely Treatment After Being in the Hospital

Community Care keeps track of how many of our members have an appointment within seven days after getting out of the hospital.  We also keep track of how many members have an appointment within 30 days after getting out of the hospital.  We track these things because we believe that members that have an appointment soon after getting out of the hospital have a better chance of staying out.  And a better chance of recovery.  It is important that you have an appointment soon after getting out of the hospital.  If you need help getting an appointment, please call the Community Care office in your county.
