Inpatient Admission Notification Requirements

Clinicians in the Hudson River Region contract are required to notify Community Care of an inpatient admission within 24 hours of admission date, or by 5:00 PM of the first business day following a weekend or holiday.

To make this process as easy possible, clinicians can either call the Hudson River Region Provider Line at any time, or use the link below to submit notification. A Community Care representative will contact you within 72 hours of submitting a notification.

• Facility Name
• Contact Name & Phone Number
• Admission Date/Time
• Consumer's Medicaid ID Number
• Type of Inpatient (Mental Health, Detoxification, Rehabilitation)
