RFQ for School Based Behavioral Health Team

Community Care Behavioral Health is requesting proposals for the development and provision of a School Based Behavioral Health Team (SBBH) Program for children, adolescents and their families in Snyder, Northumberland and Warrren Counties.  The SBBH Team Program will be implemented in identified school districts to youth who demonstrate a serious emotional or behavioral disturbance that is negatively impacting his/her ability to be educated and function in home and community settings.  Please note that services are expected to extend into the home and community settings during nonschool hours for youth who need that additional therapeutic support and intervention. 

The intention in developing the SBBH Team Program is to build a new program that is better able to respond flexibly to the needs of youth and families.   During academic year 2008-2009, three SBBH Team Programs were successfully implemented in the Northeast region in Hazleton Area School District and Wilkes-Barre Area School District.

Expansion of the SBBH Team services are currently underway in the Scranton School District and Montrose Area School District.   

Below please find the Request for Qualifications (RFQ), in addition to the SBBH Service Description, for your review and consideration.

We will be hosting an Applicants’ Conference in two different locations in the North Central Region to answer any questions providers may have about the RFQ and the process for selection of a provider.  Letters of Intent to Respond to the RFQ are due no later than October 22, 2009.  Proposals are due no later than November 5, 2009.  Details are included in the attachments.
