Medicare Part D Formulary

To assist physicians with the task of behavioral health medication coverage under Medicare Part D, we have provided a formulary listing of the Pennsylvania Part D plans. It is current as of August 2007 and will be revised shortly after 2008. Any plans that are highlighted in yellow are considered part of the low subsidy network.

For convenience, we offer the formulary in two formats. The PDF file will print correctly on two sheets of legal-size paper. You can adjust the Page Setup in the XLS (Excel) file to print it on other paper sizes:
