Notice of Privacy Practices

Community Care is committed to keeping our members’ personal medical information private and secure. The Notice of Privacy Practices explains to members how we use and share their medical information. Each member of Community Care receives a copy of our Notice of Privacy Practices at the time of enrollment. We also re-send it to all of our members every three years. Additionally, on an annual basis we include an article in the member newsletter that informs members that they can view the Notice on our website as well as call the Community Care office in their county to request a paper copy.
