We are here to help you when you need information or want to get care for:

  • An alcohol problem.
  • A drug problem.

As a member of HealthChoices, you receive Medical Assistance from the state of Pennsylvania for your health care needs. We manage your behavioral health care, which means we:

  • Answer your questions.
  • Help you find care providers near you.
  • Help you get transportation to appointments.
  • Make sure you’re receiving the right kind of services.
  • Make sure the services are covered by HealthChoices so you don’t have to pay for them.

About Your Physical Health Care

Community Care does not manage your physical (medical) health care. Your physical health plan, or Physical Health Managed Care Organization, or Access Plus takes care of this. Your Primary Care Physician makes sure your medical care is coordinated. You will use your physical health plan when you:

  • Are sick.
  • Need a check-up.
  • Need to get a prescription filled.
