Community Care will enable electronic exchange of health data that supports the Office of the National Coordinator for Health Information Technology (ONC) Cures Act Final Rule and adheres to the HL7 FHIR Release 4.0.1 specification. This will allow Community Care members to easily access their health insurance information using independent applications of their choice.

Third party applications will be permitted to connect with Community Care’s FHIR based API free of charge. Protected Health Information (PHI) is only retrievable when authorized on behalf of a Community Care member. Other types of data, like provider directories, are freely accessible to any connected application.

Patient Access API

The Patient Access API enables Community Care members to securely exchange their health plan member information, which includes personal details like name, address, phone number, and date of birth with third party applications. This API conforms to the R4 FHIR standard. To use the Community Care Patient Access API, your application must comply with the ONC Cures Act Final Rule SMART on FHIR specification. You will need to complete the application registration form.

Provider Directory API

As mandated by the Centers for Medicare and Medicaid Services, the Community Care Provider Directory API meets the specification as outlined in the Da Vinci PDEX Plan-Net Implementation Guide and conforms to the technical standard for data exchange via secure API. The Provider Directory API is publicly available and does not require an application registration or member credentials to retrieve data.
