Secure Sites for HealthChoices Providers

Community Care has two secure sites for which you must register.

HealthChoices Outpatient Registrations
This site is for all HealthChoices providers. You can use it to create outpatient registrations and review existing registrations. If you have a high volume of registrations, you can submit them as a single file using our secure file drop. If you’re already registered, you can log in now. To sign up to use these applications, please complete the forms below and mail them to the address shown on the forms.

HealthChoices and ALDA Claims

This site is for providers who serve members in HealthChoices counties or the ALDA program. You can use it to submit and review claims online. If you’re already registered, you can log in now.
