Provider Representatives

Community Care assigns provider representatives to each facility and practitioner. Names and phone numbers are provided in these documents:

How Practitioner Representatives are Assigned

Practitioners are assigned to a provider representative based on the county location of their administrative address. This address is defined by the practitioner when the credentialing or re-credentialing application is submitted.

In counties where there are higher concentrations of practitioners, Community Care has assigned a provider representative within the county based on the first letter of the last name of the practitioner. These assignments apply to all networks for which the practitioner is contracted.
