Site Visit ToolsCommunity Care’s credentialing policies require that individual practitioners who are identified as high-volume providers and non-accredited facility providers participate in on-site visits conducted by Community Care staff. These site visits include a review of physical plant safety and ADA requirements as well as a medical record review, and must be complete before initial credentialing status can be granted. Providers who have been notified that they must participate in a site visit for initial credentialing must also notify Community Care if they add or change a practice location so that a determination can be made related to the need for any additional site visits. Non-accredited facility providers may eliminate the need for an on-site visit if they provide Community Care with a current licensing report from their respective licensing entity or a CMS report. Providers who must participate in the site visits will be notified by the credentialing department in advance to schedule the visits.
