Site Visit Tools

Community Care’s credentialing policies require that individual practitioners who are identified as high-volume providers and non-accredited facility providers participate in on-site visits conducted by Community Care staff. These site visits include a review of physical plant safety and ADA requirements as well as a medical record review, and must be complete before initial credentialing status can be granted. Providers who have been notified that they must participate in a site visit for initial credentialing must also notify Community Care if they add or change a practice location so that a determination can be made related to the need for any additional site visits.

Non-accredited facility providers may eliminate the need for an on-site visit if they provide Community Care with a current licensing report from their respective licensing entity or a CMS report.

Providers who must participate in the site visits will be notified by the credentialing department in advance to schedule the visits. If you have questions in the interim please contact your assigned provider representative.
