Secure Sites for HealthChoices Providers

Community Care has two secure sites for which you must register.

HealthChoices Outpatient Registrations

This site is for all HealthChoices providers. You can use it to create outpatient registrations and review existing registrations. If you have a high volume of registrations, you can submit them as a single file using our secure file drop. Click here for instructions on how to sign up to use these applications.

HealthChoices and ALDA Electronic Claims (Provider Online)

This site is for providers who serve members in HealthChoices counties or the ALDA program. You can use it to submit and review claims online. Click here for instructions on how to sign up to use the online claims system.
